Requirements

  • For Incoming Freshmen: A Certification that he/she is a high school graduating student and that he/she has no failing grades in 4th year HS.
  • For Transferees: Transcript of Records, Transfer Credentials, or Certificate of Grades duly signed by their School Registrar.
  • For Out-Of-School Youth: Form 138-A (Report Card)
  • Two (2) copies 1" x 1" pictures
  • P212.00 non-refundable examination fee
