USEP ADMISSION TEST (USEPAT)

A.  REQUIREMENTS
  1. For Incoming Freshmen:
  • HS GRADUATING STUDENTS:
      • *Certificate indicating the applicant is a graduating HS student
  • HIGH SCHOOL (HS) GRADUATES:
      • * Photo copy of Form 138-A (Report Card).
  • TRANSFEREES:
      • * Any of the following transfer credentials: Transcript of Records, Honorable Dismissal or Certificate of Grades duly signed by the School Registrar.
     
 

2. Two (2) pcs. 1” x 1” recent ID pictures

3. P212.00 non-refundable examination fee(Subject to change without prior notice)
 

*PRESENT ORIGINAL DOCUMENTS, SUBMIT PHOTO COPY.
     
B.  PROCEDURES for Applying
 

Step 1:  Present to the University Guidance & Testing Office (UGTO) your requirements.

Step 2:  Secure a payment slip from UGTO.

Step 3:  Pay Examination Fee to the Cashier’s Office.

Step 4.  Present the Official Receipt of payment to the UGTO personnel for the release of Application Form.

Step 5:  Submit duly accomplished form to the Guidance Office. Only accomplished forms with complete requirements will be processed.

Step 6: THE APPLICANT SHOULD TAKE THE ADMISSION TEST ONLY ONCE.
     
 

Filing for application is now GOING ON (on a first-come-first serve basis)
