\n1. Photocopy (with Certified True Copy) of Report Card obtained during Grade 11 (GPA of 1st and 2nd Semesters) and Grade 12 (GPA of 1st Semester)
\n2. Two (2) pieces recent passport-size ID pictures with name tag
\n3. Photocopy (with Certified True Copy) of ITR OR Certificate of Low Income OR Certificate of Indigency (You may request this from your barangay)<\/p>\n
