Photocopy (with Certified True Copy) of Report Card obtained during Grade 11 (GPA of 1st and 2nd Semesters) and Grade 12 (GPA of 1st Semester)<br \/>\n2. Two (2) pieces recent passport-size ID pictures with name tag<br \/>\n3.
