To avail of the free dental and medical certification, please submit the results of the following laboratory tests in a long brown envelope to the USEP-CDM Mintal Clinic:
– Complete Blood Count (CBC) with Platelet Count
– Blood Typing
– Chest X-ray
– Urinalysis
