The visit of Medical and Dental doctors in our campus will be on November 23, 2022. Students who are not yet done with their Physical Examination/Medical Certificate are welcome to visit USeP-CDM Clinic. Kindly bring the following:

– Laboratory results
– 1×1 ID picture
– Health Declaration Form with signature of your parents/guardian
– Photocopy of your vaccination ID
– All the documents must compile in a long brown envelope.
