·         Data Privacy and Non-Disclosure Form

·         Personal Health Declaration Form (for undergraduate applicants only)

·         Workplace video presentation

·         Recommendation letter from the immediate superior

(For applicants with proven competencies but currently unemployed, please submit a notarized service record or other corroborative documents such as Income Tax Return (ITR), SSS, PhilHealth and Pag-IBIG records.)

·         Certificates of:

§  Employment with job description

§  Recognitions/Awards received

§  Membership in professional organizations

·         Proficiency Certificate, from any of the following:

§  Professional Regulation Commission

§  Professional Organizations

§  Government Regulatory Boards

IMPORTANT NOTE

1.    All application documents and attachments to be submitted must be duly notarized.

2.    The original copy of the following documents should be submitted to the ETEEAP Office or through mail courier*.

-       PSA Birth Certificate

-       TOR, Form 137

-       Honorable Dismissal

-       2pcs 2x2 Picture

-       Data Privacy and Non-Disclosure Form*

-       Personal Health Declaration* (For undergraduate applicants only)

3.    Arrangement of application documents and attachments through electronic filing:

Ø  Create a google drive folder entitled “SURNAME, INITIALS_TARGET PROGRAM” (e.g., PADILLA, DM_MPA-OS)

Ø  Within the google drive folder, make a separate folder for the basic requirements entitled “REQUIREMENTS”, to wit:

-       Filled out ETEEAP Application Form

-       Filled out Competency Outcomes Form

-       NSO/PSA Authenticated Birth Certificate

-       School Credentials (e.g., TOR. Form 137, Honorable Dismissal)

-       NBI clearance or Barangay Clearance (latest)

-       Resume or PDS

-       Recommendation letter from the immediate superior

-       Filled out Data Privacy and Non-Disclosure Form

-       Filled out Personal Health Declaration Form

-       Workplace Video Presentation

Ø  In filling out the Competency Outcomes Form, please refer to the program prospectus. You should include a shortened URL or link for the specified document on the Evidences column. (Click here to see example)

Ø  Make a separate folder for the certificates and evidences for the competencies entitled “ATTACHMENTS”

4.    Share the google drive folder to: eteeap@usep.edu.ph as “editor”.
